To refer a patient:

1. Download the referral form specific to the therapy. If there is not one available, download the "Generic Referral Form."

2. Complete the referral form

3. Fax the completed referral form along with requested documentation to (866)542-8631

Upon receipt, our team will handle the infusion therapy treatment process and contact your office should any issues arise and keep you updated of the insurance authorization process.
